دستور العمل ارسال مدارک درمانی جهت اخذ هزینه از کانون سردفتران و دفتریاران

احتراماً ، با توجه به اجرای طرح تفکیک هزینه های بیمارستانی از سرپائی و پاراکلینیک ، بمنظور تسهیل و تسریع در امر خدمات رسانی لازم است از تاریخ نامه ، واحد پذیرش نسبت به جداسازی پرونده ها از یکدیگر به شرح ذیل اقدام گردد.

هزینه های بیمارستانی
هزینه های دندانپزشکی و عینک
سایر موارد سرپایی و پاراکلینیک

در ابتدا و هنگام پذیرش هر یک از کارشناسان پذیرش نسبت به بررسی ، تفکیک مدارک و بر حسب موارد فوق الذکر نسبت به دسته بندی پرونده ها اقدام نموده بصورتیکه در هر پوشه یک کپی از نامه سردفترنیزالصاق گردد.

پس از این، بخش پذیرش نسبت به ارائه و ارجاع پرونده ها به کارشناس مربوطه با ذیل انجام پذیرد:

الف: مدارک لازم جهت اخذ هزینه های پاراکلینیکی + سرپائی:

نامه سردفتر ، دفتریار و یا سایر
دستور پزشک معالج - اصل قبض رسیدمعتبر صورت هزینه - اصل یا کپی جواب اقدامات انجام شده.(هزینه های زیر2.000.000 ریال با قبض رسید و از یکی از دو مورد دستور و یا کپی جواب قابل پرداخت است.)
در خصوص هزینه آزمایش و دارو ریز موارد و درج قیمتها بصورت خوانا الزامیست.

ب:مدارک لازم جهت اخذ عینک:

نامه سردفتر ، دفتریار و یا سایر
دستور چشم پزشک و یا اپتومتریست
اصل قبض رسید معتبر عینک

ج: مدارک لازم جهت اخذ دندانپزشکی:

نامه سردفتر،دفتریارو یا سایر
اعمال انجام شده در برگ معتبر بهمراه قیمت توسط دندانپزشک
OPG یا پری اپیکال بر حسب کار انجام شده

د: مدارک لازم جهت اخذ هزینه های بیمارستانی:

نامه سردفتر ، دفتریار و یا سایر
اصل صورتحساب مرکز پزشکی (شامل صورتحساب بیمارستانی ، صورتحساب پزشکان ،…).
فرانشیز دریافتی از بیمار بر مبنای مندرجات در معرفینامه دقیقاً مشخص گردد.
گواهی تعداد ویزیت و مشاوره انجام شده.
گواهی پزشک بیهوشی (در صورت انجام عمل جراحی ) باقید پایه و مدت بیهوشی.
گواهی پزشک معالج و جراح و تائید کمک جراح توسط جراح با ذکر اقدامات انجام شده.
برگه شرح عمل.
گواهی مبالغ دریافتی از بیمار بابت مازاد تعهدات ، همراه ، اتاق خصوصی ،لوازم بهداشتی و… (با ریز اقلام).
در صورت انجام درمان طبی، ارائه برگه سیر بیماری ، دستورات پزشک و خلاصه مدارک درمانی الزامی است.
صورت ریز کلیه نسخ داروئی.
صورت ریز وسائل مصرفی اتاق عمل با مهر و امضاء پزشک جراح و حسابداری (فاکتور خرید جهت اقلامی که نیاز به فاکتور خرید دارد) و در خصوص وسایل یکبار مصرف الصاق بر چسب مشخصات بر روی برگ شرح عمل الزامیست.
قبوض رسید و جواب پاتولوژی ، رادیوگرافی ، آزمایشگاه ، ام.آر.آی ، سی تی اسکن ، اکو ، تست ورزش ، هولتر ، آنژیوگرافی و …
در صورت انجام فیزیوتراپی گواهی تعداد جلسات و تاریخ آنها و اقدامات انجام شده در هر جلسه.
درصورت انجام سنگ شکن ارائه گواهی پزشک معالج با ذکر مرحله انجام شده (1و2و3) ضروریست.
برای عمل رفع عیوب انکساری چشم گواهی پزشک معالج مبنی بر میزان عیب انکساری چشم و برگ اتورفراکتومتر ارایه شود.
در صورتیکه مرکز پزشکی طرف قرارداد با سازمان بیمه خدمات درمانی ، سازمان بیمه تأمین اجتماعی و یا سایر بیمه گران باشد میزان پرداختی بیمه گر اول مشخص شده باشد.
هر گونه قلم خوردگی و لاک گرفتگی توسط مقام صادر کننده مجدداً مورد تائید قرار گیرد.
جهت محاسبه دارو و اقدامات پاراکلینیکی ارایه برگ سفید دفترچه بیمه و برگ آبی دفترچه جهت محاسبه ویزیت ( و درج ویزیت در محل مشخص توسط پزشک) الزامی است.

نسخه های مربوطه می بایست ممهور به مهر پزشک و دارای تاریخ باشد.
